The Department of Labor issued a new FMLA poster and updated FMLA forms. The following is a list of the forms.
WH-380-E Certification of Health Care Provider for Employee’s Serious Health Condition
WH-380-F Certification of Health Care Provider for Family Member’s Serious Health Condition
WH-381 Notice of Eligibility and Rights & Responsibilities
WH-384 Certification of Qualifying Exigency For Military Family Leave
WH-385-V Certification for Serious Injury or Illness of a Veteran for Military Caregiver Leave
Additionally, the DOL issued a new poster. It provides a summary of the FMLA and informs employees how to file a complaint. All covered employers must display the new notice poster in a conspicuous place where employees and applicants for employment can see it. The deadline is March 8, 2014. Electronic posting also is permitted to satisfy the posting requirement, as long as it otherwise meets the requirements of the regulations.

NEW 1-9 FORM
Additionally, USCIS issued a new I-9 form to be used by employers. Using an expired form may trigger fines.
